2019 Healthcare Scholarship
Guidelines for Application
Applications Due to the Executive Director on or before March 25, 2019

The Paulding County Hospital Foundation Scholarship is to be awarded to qualified candidates annually to Seniors from Antwerp High School, from Paulding High School, and from Wayne Trace High School, who are planning to pursue an advanced degree in healthcare. Home-schooled students will also be eligible and will be placed in one of the three districts according to their addresses. The scholarship is to be used for the financial assistance of a student in post-secondary healthcare education.
The criteria and selection process are as follows:
1. The award will be made jointly to the student and to the registrar of the post-secondary educational institution.
The selection committee will take a combination of the following factors into consideration (not in order of importance):
2. The recipient must have at least a B average.
3. The recipient must intend to continue his/her education in a healthcare field (for example: nursing, medical school, laboratory technician, radiology, respiratory technician, physical therapy, etc.)
4. The successfully completed application packet will include:
5. A representative of the PCH Foundation shall present the scholarship.
6. The amount of the award for students is $1,000 per school. However, if there are no applications from a school, or the applications submitted are not of acceptable quality, the Foundation may choose not to award a student scholarship to that school for that year. All scholarships are a one-time award.
Applications MUST be received by the Executive Director on or before March 25, 2019. Applications received after this date will NOT be processed.

Timeline for 2019 PCHF scholarship committee:
February 18 Applications delivered to high schools
March 25 Applications due to be returned by students
April 4-5 Students notified who qualify for Interviews
April 10 Student Interviews with selection committee
April 11-12 Award winners notified by Executive Director
April 22, 2019 Foundation Spring Member Banquet
Scholarship Winners are expected to be present: 5:30 pm at the Paulding Eagles Lodge. Parents of winners are also the guests of the Foundation for the banquet.

- Intended field and knowledge/experience in that field
- Shadowing or practical exposure to healthcare field desired
- GPA and transcript information
- High school ranking
- Course selections in relation to healthcare field desired
- Referral questionnaires
- School attendance
- Completeness of application
- Extra curricular activities
- Quality of the Interview (for those qualifying to Interview round)
2. The recipient must have at least a B average.
3. The recipient must intend to continue his/her education in a healthcare field (for example: nursing, medical school, laboratory technician, radiology, respiratory technician, physical therapy, etc.)
4. The successfully completed application packet will include:
- The completed application form
- A student evaluation form completed by the Science teacher of the applicant’s choice.
- A student evaluation form completed by the Guidance Counselor
- A high school transcript through seven semesters
5. A representative of the PCH Foundation shall present the scholarship.
6. The amount of the award for students is $1,000 per school. However, if there are no applications from a school, or the applications submitted are not of acceptable quality, the Foundation may choose not to award a student scholarship to that school for that year. All scholarships are a one-time award.
